"Unlike other survival-horror titles, in Painkiller it's the monsters that should be afraid of you." This quote comes from the game description in this site (which means it probably comes from the official site or press release).  Jesus, who writes these things!  Painkiller has nothing to do with survival-horror.  It is a gothic-themed straightforward FPS in the footsteps of the original Quake.  In fact, if you liked Quake I it's almost certain that you're going to love this game.  The graphics are gorgeous, the enemies are decidedly undead or demonic and the weapons are...  well both gothic and out of this world.  Highlights include the bladespinniner-energy-harpoon-ComeOverHere Painkiller, the powerfully satisfying stake gun and the (drumroll) chaingun-rocket launcher combo!  Combine all this with excellent ragdoll (or rather zombie bonebag) physics, a solid heavy metal soundtrack and an eerily gothic setting and that's the game in a nutshell.   
Painkiller is not a game that threatens Far Cry or the long-awaited Half-Life 2.  No dynamic objectives, no story in-game, no NPCs, no alternate routes.  Doors slam shut behind you, limiting you in relatively small areas and you have to kill everything in order to proceed.  It's not a thinking man's FPS, but a perfect way to blow off some steam and have fun killing perfectly hateable enemies in a surrealistic environment that depicts Purgatory under invasion from Hell.  And you get to clean the mess since Heaven does not want to "risk open war".  Splendid.  Oh, did I mention that you have to dispatch the four Generals of Hell and that they are the hugest bosses you've seen in 3D this side of Serious Sam?
